Intralipids Therapy During IVF Treatment
Immune disorders play a significant role in patients with recurrent implantation failure in IVF and elevated Natural Killer cell count and/or activity is one of the most likely underlying causes of this condition.
Intralipid is the brand name for a 20% fat emulsion for human use. It is administered via an intravenous (IV) drip. Chemically it is a synthetic emulsion of soy bean oil, egg yolk, water and trace amounts of peanut oil.
Intralipid has been used with IVF treatment to help women who suffer from either recurrent miscarriage or repeated implantation failure following embryo transfer.
In a number of scientific studies it has been seen that intravenous administration of intralipids improved the rate of live births following IVF treatment in women who previously had recurent failures. Several studies have proven suppression of NK cell numbers and cytotoxicity by usage of intralipids. The results of a small study suggested that Intralipid treatment could be used successfully as a therapeutic option to modulate abnormal NK activity in women with reproductive failure.
From a human clinical trial data of recurrent miscarriage sufferers, it was concluded that Intralipid even in small doses could be an effective antiabortion treatment. The number of patients in the published study is too small for the required degree of precision. Intralipid was highly effective in preventing abortion in mice.
Natural killer cell testing is currently practised widely, and there are studies indicating potential benefit in terms of targeting women with repeated reproductive failure for immune therapy.
Intralipid treatment has the advantage that it is relatively inexpensive and, not being a blood product, free of the more serious side effects and risks associated with IVIG.
Timing of infusions
Intralipid infusions are normally carried out 7-10 days before embryo transfer. If a positive pregnancy test ensues further infusions should be carried out on a 4 weekly basis until 12 weeks of pregnancy.
Adverse Reactions
Side effects to intralipid infusion are extremely rare. The most common reported side effect being a cold sensation in the infusion arm.
